For your convenience, these forms are located under our patient forms tab to download, complete and bring prior to your appointment, or at the time of your appointment.

Please note the questions we ask will provide our staff with essential data on your health and medical history. You will also be asked questions concerning your contact, insurance and billing information. There may be times when we will need to contact you, so please provide us with complete information, including a cell number and email address. Periodically, you will be asked to update your patient file in order to help us provide the best care and service.

Prescriptions this website

We Prescribe most our of prescriptions directly to your pharmacy. Refills of existing prescriptions will be issued during normal office hours. In addition, since it is difficult to prescribe or authorize medications after office hours, we ask that you have your pharmacist call our office during our business hours.

Financial and Billing

As stated in our Texas Cardiology Associates of Houston office policies, we have made every effort to ensure our fees reflect the usual and customary fees for our general area. As with any professional service, our providers appreciate prompt payment for their services. Texas Cardiology Associates of Houston office policies state that we want to provide quality medical care at a reasonable cost and will discuss our fees and billing policies with you as needed. In addition, we urge you to discuss any personal financial problems with us so payment arrangements, acceptable to both parties, may be arranged. We participate in most insurance plans available in the area. Should you have any questions about whether we participate in your plan, a member of our office staff will be happy to help you.

As a service to our patients with insurance, our billing department will file all claims on behalf of the patient. Your health insurance is designed to help you meet the cost of medical services. In most cases, it is not designed to pay total fees. Each insurance policy and healthcare provider offers its own terms.

If you are unsure of your policy coverage, please call the 1-800 number listed on the back of your insurance card or visit your insurance company’s website. Please keep in mind that we cannot hold another party responsible for payment of services that you obtain. This applies to insurance companies, as well as to other parties, who you believe may be responsible for payment of your bills. We ask you to be responsible for the full payment if you are not covered by insurance plans for which we are providing. If you would like to pay your bill or if you have any questions regarding billing information please call our office or email us at Billing@tcahouston.com.
